Plans For Independent Living
When you enter our Life Plan Community as an independent living resident, you will sign an agreement stating you do not own your home at Heritage Pointe but have use of it as long as you are able to live independently. Along with a one-time entrance fee, you’ll pay a monthly service fee which provides a totally carefree, maintenance-free lifestyle and includes:
- Utilities.
- Property taxes.
- Access to a wide variety of community services, amenities, and activities.
- Home and grounds maintenance.
- Repair or replacement of appliances provided by the community.
- Priority access to Assisted Living, Memory Care, Skilled Nursing and Rehabilitation should the need arise.
- The opportunity to apply for provision of care if your financial resources become exhausted through no fault of your own.
Heritage Pointe of Warren offers two entrance fee plans from which to choose.
Refundable Plan
With our refundable plan, incoming independent living residents pay a higher entrance fee, but a percentage of that entrance fee is refunded (on a prorated basis determined by length of occupancy) to residents should they leave or to their estate.
Non-Refundable Plan
Our non-refundable plan features a lower initial entrance fee, but none of the fee will be refunded should you no longer reside at the community.
Plans for Advanced Levels of Care
Heritage Pointe of Warren allows you to enter our community at the level of care most appropriate for you. Each level of care offers choices so you can tailor a price structure appropriate for you.
Assisted Living and Memory Care
Residents joining Heritage Pointe of Warren at the care levels of Assisted Living or Memory Care have the choice of entering with no entrance fee or can pay an entrance fee with the advantage of a lower “monthly-pay” rate. Either way, the total rate paid is ultimately determined by the specific care required, so residents never pay for care that’s not needed. As needs change, care services can be added with an accompanying price change.
For Assisted Living and Memory Care, Heritage Pointe of Warren accepts:
- The Medicaid waiver program.
- Some long-term care insurances with an assisted living benefit.
- VA benefits.
Skilled Nursing
Similar to Assisted Living and Memory Care, a resident joining Heritage Pointe of Warren at the care level of Skilled Nursing has the choice of entering with no entrance fee or can pay an entrance fee with the advantage of a lower “monthly-pay” rate. The total rate paid is ultimately determined by the specific care required and whether the residence is private or semi-private.
For Skilled Nursing, Heritage Pointe of Warren accepts:
- The Medicare replacement plan for short-term stays.
- Medicaid for long-term stays, contingent on application and acceptance by the state of Indiana.
- Long-term care insurance providers, as well as insurance providers for short-term stays for rehab and skilled nursing.
