Heritage Pointe of Fort Wayne Earns CMS High Performing Facility Designation
October 15, 2026
The skilled nursing home of Heritage Pointe of Fort Wayne senior living community has earned the High Performing Facility designation from the Centers for Medicare & Medicaid Services (CMS). This distinction is awarded to approximately 12% of nursing homes nationwide based on criteria that include staffing, health inspections and other quality indicators. Heritage Pointe of Ft. Wayne is a Life Plan Community, also known as a CCRC, providing independent living, assisted living, memory care, rehabilitation, and skilled nursing on campus.
Introduced nationally on October 8, 2026, the High Performing Facility designation is represented by a trophy icon on the CMS Nursing Home Care Compare website. It identifies nursing homes that meet additional performance requirements beyond those reflected in the traditional Five-Star Quality Rating System.
Although the designation is not a sixth star or a separate numerical rating, it recognizes facilities that meet CMS criteria for a more focused, risk based routine inspection process. The criteria include a five-star overall rating, at least three stars for staffing, a qualifying health inspection record and accurate staffing and resident assessment data. Additional requirements address survey history, safety related citations and other performance measures.
“We are very proud to receive this prestigious designation,” said Chod Boxell, Chief Executive Officer of Heritage Pointe Communities. “This recognition reflects the dedication of our entire team and, most importantly, our commitment to providing residents with high-quality care in a safe, supportive environment. We see this as a reflection of the trust our residents and their families place in us every day, and it motivates us to continue raising the standard of care we provide.”
The distinction provides residents and families with another way to evaluate nursing home performance when comparing care options. CMS recommends considering the overall rating alongside individual measures, available services and the specific needs of prospective residents.
